About the Role
The Re-entry Specialist will be responsible for conducting an intake with each new client to assess the client’s non-legal needs, including housing, case management, employment, family reunification services, transportation, communication technology support, and education.

Requirements
Duties/ Responsibilities:
Coordinates mentor contact and instruction to those who have been formerly incarcerated.Â
Assists clients in accessing a variety of resources to address their reentry needs.Â
Creates a reentry service plan for the client, with a focus on appropriate resources.Â
Connects the client to identified community providers and determine the appropriate referral pathways. Â
Assist clients with resources that are within their scope of practice.Â
Conducts employment training sessions with individuals who have been formerly incarcerated.Â
Distributes transportation passes to those formerly incarcerated individuals who require transportation for class work or job interviews.Â
Assists individuals involved in the program to complete potential requirements for payment of fines, simple legal issues, and housing.Â
Collaborates with service providers throughout the state to establish and maintain relationships and referrals.Â
Creates and maintains a system to track referrals and program outcomes.Â
Completes all reporting requirements necessary for the re-entry program.Â
Develops a file documentation system for those individuals enrolled with the re-entry program.Â
Performs other related duties as assigned.
Required Skills/ Abilities:
Desire to work collaboratively with the ability to work independently.
Strong drive and initiative with the ability to manage time well in an environment of many concurrent assignments and deadlines.
Accurate and detail-oriented.
Knowledge of software programs including Microsoft Office Suite, Google Suite, and the desire to learn and develop skills within other software programs.
Ability to organize electronic files and email efficiently so that information is consistently saved in locations that are easy for staff to access.
Ability to use case management and electronic file storage systems to take case notes, chart referrals, and save case management information.
Pass backgrounds if necessary for correction facilities
Education and Experience:
High school diploma or equivalent required.
Demonstrated cultural competence and sensitivity in working with diverse clients, communities, and colleagues.Â

Physical Requirements:
Must be able to lift up to 15 pounds at times.
Must be able to go out to various locations for outreach.
